when you leave care, you will have all your relevant ID documents – driving licence, passport, birth certificate, Settled Status Documents, deed poll (if required)
we aim to build a positive and long-lasting relationship with you so you can help us understand your journey and support your wellbeing
we will work with you and alongside you; on your plans and future goals to help you achieve these. You will have a Pathway Plan until you are at least 21
we will help support you to understand your past and your journey. We are here to help you access your files and share this with you sensitively and with support
we will help you maintain your family links and relationships with people important to you, so we can ensure you have two significant relationships when you leave care
where it is appropriate, we will support with bus and travel passes in Derbyshire or up to £100 per year
we will listen to you and help promote your voice and vision for the service, through Derbyshire’s care leaver council ‘Next Generation’'
you can join and use any district council Derbyshire leisure centre for free to keep fit and healthy – with a free gym pass
we will support you to feel part of your community and help you access local groups you may be interested in. We will help you create roots in your community, to help you feel you belong
we will keep in contact with you until you are 25 if you need this. After 25, we have an ‘Always There’ approach
we support you when you start your own family and have children of your own, we are here to help you when starting out
you will receive up to £35 on your birthday until you are 21. For your 18th and 21st birthday, your Leaving Care Worker will take you out to celebrate your big day
we will ensure you have a laptop if you need this and access to the internet
if you live in semi-independent supported accommodation and pay a service charge, as a child in care, you can be entitled to £500 per year cultural fund to help support your cultural needs or hobbies. This is £100 per year when you are a care leaver up to 21
