Leaving care service - health and wellbeing

Our promises to you for health and wellbeing.
  • both our offices operate an open-door policy in office hours – we will always have food and facilities available to you
  • you will receive a health history summary when you leave care (unless there are exceptional circumstances)
  • we support you to access community health teams, GPs, adult social care, adult support services, young adults pathway and agencies like Trent PTS
  • we work closely with Derbyshire Recovery Partnership who have a wide support service for young people who may be struggling with alcohol or substance misuse
  • we will assist you with accessing the right health services and support when you need it
  • Kooth are available to you all hours, when you may need this support the most
  • we help you have access to your health history from birth from your GP
  • we do regular events and trips out, (seaside visits, events at our offices). Ask your worker for details or check out Facebook/Twitter
  • help to access physical and mental health support, activities and groups from our partner agencies
  • with your consent we can work with ‘Action for Children’ they help us help you, giving us advice and support when we and you need it
  • we operate a care leaver mentoring scheme, where you can talk to others with shared experiences
  • understand your life story, we can support you with accessing your information to understand your journey
  • identity, culture, sexuality, religion, denomination, all these matter to you and us, we are here to help you access support to negotiate through these sensitive and delicate areas
  • access to LGBTQ+ groups across Derbyshire, in all localities
  • leaving care have football groups set up north and south – you can join in too
