Housing related support services

We fund housing related support services to enable people drawing on care or support to achieve or maintain independent living in the community. More than 24,000 people per year in Derbyshire can be supported through these services.

A wide range of people receive housing related support services, and this includes people without a diagnosis or recognised disability.

We fund practical housing support services to help people live independently:

• assistive equipment and home adaptations
• assistive technology and community alarms

We also commission a range of accommodation support services:

• Extra care housing
• independent living services can be delivered to service users living in any type of home - private, rented or housing authority - and can include crisis support
• accommodation based support services for people who need temporary accommodation to enable them to get the appropriate level of support
• the Derbyshire Home from Hospital Support Service provides practical support for vulnerable people leaving hospital, or those living at home but at risk of being admitted

The support available can consist of:

• setting up and managing your home
• keeping your home safe
• organising adaptations
• dealing with forms and letters
• managing your money or debts and arrears
• access to training or employment
• developing independent living skills
• putting you in touch with other services
